Admins can create custom feedback templates that are available to employees to use when requesting Feedback.
Create a new template
- Navigate to Admin > Feedback > Templates.
- Select Create template.
- Enter a template name.
- Add your prompts in the text box.
- Select Create template to save.
Note: Templates cannot be assigned to specific user groups and will be available to all employees.
Edit or Delete a Feedback Template
- Navigate to Admin > Feedback > Template.
- Next to the Feedback template, select the ellipsis (...).
- Select either Edit or Delete.
Use a custom feedback prompt
- Navigate to Home > Give or request feedback -OR- Home > Feedback.
- Within the Feedback tool, navigate to the Request feedback tab.
- Under Who do you want to ask for feedback?, select employees at your company by name or externally by providing an email address.

Under Who’s the feedback about?, select either who the feedback is associated with. Based on your permission levels you may be able to select from:
- All users
- Direct reports
- Indirect reports
- Yourself
- (Optional) Under What would you like feedback on?, choose to:
- Select the desired feedback template.
- Note: Templates can be edited once they have been added to the text box and there is no limit to the number of prompts added.
- Write your own feedback request in the text box.
- Select the desired feedback template.
- (Optional) Add a growth area or competency.

Choose your share settings.
- Manager only: feedback is private and will go directly to an employee's manager
- Private: feedback is visible to only the recipient
- Private + manager: feedback is only visible to the recipient and their manager
- Select Request feedback.
